Proper referral system is essential to ensure the equitable distribution of health system particularly in low resource country. Although national health policy has provision to maintain a standard referral system, these facilities are often poorly utilized by the patients leading to overcrowding of the tertiary centers and unnecessary economic burden to care seekers. The lack of knowledge among the people about the existing referral system may a reason behind this underutilization of the system. Thus, the current study has been designed to determine the knowledge of the people about the existing referral system by assessing awareness and perception of the patient or their attendants toward referral system in our health care delivery system. It will be a descriptive cross-sectional study and will be conducted among 448 patients or their attendant registered in out- or in- patient department of Dhaka medical college hospital and Bangabandhu Sheikh Mujib Medical University for six months period following approval of this protocol.
